Before the plan anniversary (January 1), or within 30 days of your original employment date with Staff One, Inc., you may select a portion of your salary to be deducted from each paycheck for the Flexible Benefit Plan. Employees who wish to continue their participation in the Flex Plan must re-enroll every year prior to the January 1 effective date. Simply follow the steps below to enroll.

  • Using the worksheet provided, calculate the amount of eligible expenses you plan to have for your entire plan year.
  • Please use our employee calculator to assist you in calculating savings.

  • Fax your completed Enrollment Form and Direct Deposit Authorization to the Staff One Benefits Department at (580)920-0863.
